Yearly visits with a primary care provider are a good time to raise any medical concerns, discuss changes to your family’s medical history, and talk through simple steps you can take to optimize your health and wellness.
Diagnosis and treatment of minor illnesses
If an acute illness or minor symptoms have you down, we provide expert care that gets you back to feeling your best. Examples of symptoms we diagnose and treat during a sick visit include: Sore throat, bad cough, fever, constipation, diarrhea, rashes, swelling, fatigue.
Primary care offices should not be considered alternatives to emergency care. If you are experiencing life-threatening symptoms, get to the nearest emergency room.
Management of chronic conditions
A chronic medical condition is one that lasts for a long period of time. Chronic conditions often impact a patient’s quality of life, may require ongoing medical treatment to manage, and sometimes have no definitive cure.
Should the need arise for specialized care, General Physician will quickly connect patients with the right expertise, within our own expansive network of specialists.
Health screenings and diagnostic testing
Health screenings are tests that help detect medical problems as early as possible, oftentimes before symptoms even appear. In general, the earlier an issue is identified, the easier it is to be treated.
Whereas diagnostic testing is a test used to diagnose a person’s medical condition, usually based on signs and symptoms that have already appeared.
Health screenings and diagnostic testing are typically coordinated through your primary care provider’s office, but patients may need to see a specialist for the test itself. Examples are scheduling a colonoscopy with a colorectal or gastroenterology provider or going to an imaging center for an imaging test (such as a CT scan or MRI).

Individualized nutrition counseling, diabetes education, and other resources are available to General Physician primary care patients. Your provider will collaborate with our team of registered dietitians (RDNs) to tailor a care plan to achieve your unique health goals.

Routine vaccinations are recommended immunizations that help protect communities against the spread of infectious diseases.
Our primary care providers can administer routine vaccinations during an in-office visit, and we can also address any questions or concerns you might have about immunization.

Adult Primary Medicine
Hertel-Elmwood Medicine Center is your neighborhood center for the medical care you need, where and when you need it. Our providers work as a team to ensure you have a single place to go for diagnosing illness, annual check-ups, managing diseases, lab work, heart and foot care, skin biopsies, and more. When additional care is needed from a specialist outside the Center, our team will help you coordinate referrals and appointments.
As an approved Patient Centered Medical Home (PCMH) office, our goal is to earn your trust by providing you – and every patient we see – with high-quality care, education, and resources to prevent illness and treat your medical needs. Our health providers are fluent in 16 languages (English, Arabic, Bangladeshi, Chinese, Dutch, French, Gujarati, Japanese, Nepali, Hindi, Persian, Punjabi, Russian, Spanish, Tamil, and Urdu), and focus on building respectful relationships that address individual cultures, values, and preferences of each patient.
The Center is also an LGBTQ Community Committee Outreach Site “Safe Place” for individuals experiencing greater exposure to violence, homelessness, and other disparities.
