What is a clinical pharmacist?

When you think of a pharmacist, you may picture receiving a prescription at a pharmacy. But clinical pharmacists differ from other types of pharmacists, by practicing out of our primary care offices. 

Clinical pharmacists do not dispense prescriptions but assist with deciding which medication is best for you. They work directly with you and your care team to help manage your health conditions and medications. This includes ensuring cost-effective treatment is being prescribed.

Hospital-based pharmacy

Kaleida Health also offers an outpatient pharmacy at Buffalo General Medical Center that serves all four Kaleida Health hospitals. The pharmacy accepts most major insurances and specializes in care discharge medications, prescription compounding, pediatric compounding and hard-to-find prescription medications.

Frequently asked questions

Find answers to some of the most common questions about clinical pharmacy services at Kaleida Health. 

Any patient of a Kaleida Health provider can receive pharmacy services. Depending on the clinic, these services may be offered in person, remotely or both.

You must first be a patient of a Kaleida Health provider. Every time you meet with a clinical pharmacist, a record of your pharmacy consult is sent to your Kaleida Health provider. If you are a Kaleida Health primary care patient, we may be able to schedule your primary care and pharmacy appointments together, depending on availability. A pharmacy consult may also be scheduled separately from a primary care appointment.

Every patient is different, which means each consultation is customized to your needs. You can expect to discuss your medical conditions, any new diagnoses and your medications.

Plan to bring the following in addition to anything else requested by your provider:

  • An updated medication list or your medication vials
  • Any data that would help assess current medications, such as blood glucose or blood pressure readings

Conditions managed with the help of a clinical pharmacist include, but are not limited to, asthma, COPD and other lung diseases; diabetes; high blood pressure; high cholesterol; nicotine dependence; polypharmacy; and geriatric de-prescribing.

Our clinical pharmacists can assist with training, setup and monitoring of many medical devices including, but not limited to, insulin pumps, blood glucose meters, insulin pens, inhalers, on-body infusers and injectable medications.
