About Primary Care Pediatrics

The Primary Care – Pediatrics program at Golisano Children’s Hospital of Buffalo provides high-quality, accessible medical care to support children’s health and development from infancy through young adulthood, in partnership with UBMD Pediatrics and the Jacobs School of Medicine and Biomedical Sciences at the University at Buffalo.

Who we serve

Families may benefit from pediatric primary care services if their child needs:

Special Needs Center at Niagara Street Pediatrics

Primary care for children with medical complexity, including developmental, behavioral, and chronic medical conditions.

Our multidisciplinary team provides coordinated, family-centered care and ongoing support.

Coordinated services

Our team serves as a consistent point of access to the health care system and works closely with specialists to develop shared care plans.

Effective care coordination may reduce unnecessary office visits, testing, emergency department visits, and hospitalizations.

Patients may also be seen in regular primary care clinics for sick or urgent visits.
 

Intake criteria

Children may be eligible if they have:

  • Chronic conditions involving five or more organ systems or specialists
  • Technology dependence (feeding tubes, tracheostomy, oxygen)
  • Medical fragility (evaluated through referral review)

Youth Link

A comprehensive support program for youth and young adults ages 13–24.

Services include:
 

  • Sexual health care
  • HIV/STI prevention and treatment
  • Mental health services
  • Housing and insurance referrals
  • PrEP (pre-exposure prophylaxis)

Our pediatric primary care team includes board-certified physicians and advanced practice providers affiliated with Golisano Children’s Hospital of Buffalo and UBMD Pediatrics.

Support staff

Care coordinators, nurses, social workers, dietitians and administrative staff work together to support patients and families.
