Golisano Children’s Hospital of Buffalo Healthy Kids is a New York State Department of Health–approved Children’s Health Home Care Management program designed to support families whose children are experiencing physical, mental or behavioral health challenges.
Through Golisano Children’s Hospital of Buffalo Healthy Kids, families are connected to medical, behavioral health, educational and community services through personalized care coordination and advocacy.

What is the Children’s Health Home program?
A Children’s Health Home is a care management program for Medicaid-eligible children who meet chronic condition eligibility criteria.

How to Refer a Child
Children may be referred to Healthy Kids by parents, caregivers, schools, healthcare providers or community organizations.
To make a referral:
