What is Primary Care?
Primary care providers are general medical practitioners who specialize in keeping patients healthy and feeling their best.
Most primary care providers recommend an annual visit to your doctor’s office to successfully manage your care.
At a primary care office visit, you can expect:
- A physical exam and verbal discussion of how you’ve been feeling
- Conversation about changes to your medications or family medical history
- Management of chronic conditions
- Diagnosis and treatment of minor illnesses (as needed)
- Instructions for recommended bloodwork, imaging and diagnostic testing
- Routine vaccinations
- Focused support for topics like diabetes, high cholesterol, nutrition and more!
The Care We Provide
Clinical Pharmacy Services
By providing care at our primary care offices, our clinical pharmacists work directly with patients and their primary care providers to successfully manage health conditions, medications and select medical devices.
Home Visits
There are times when a patient may not be able to make it to the office to see their primary care provider. Our Home Visits team provides Enhanced Annual Visits (EAVs) in the home for patients identified as high-risk and/or homebound.
Hospitalist Services
Wishing you had the support of a primary care provider when you or a loved one is admitted to the hospital? A hospitalist will become your primary point of contact at Kaleida Health facilities – to monitor your condition, direct your treatment and visit you daily.
Nutrition Services
Adult primary care patients are eligible to receive support from our registered dietitian-nutritionists for nutrition counseling, diabetes education and more!
Patient-Centered Medical Home
Patient-Centered Medical Home is a team approach to coordinate your total health care. Please reach out to your Kaleida Health primary care provider’s office to learn more about PCMH.
