What to Expect When You Arrive
The Admissions Departments at all Kaleida Health hospitals are located in or near the main lobbies. An admissions associate will complete your admission paperwork, obtain or confirm your insurance and other information, collect a deposit if necessary, give you your hospital ID wristband, and provide a patient information packet that includes a patient’s bill of rights and other important documents.
What to Bring
-
Your insurance card
Bring your current health insurance card so we can verify and confirm your coverage.
-
Photo identification
A valid photo ID helps us confirm your identity at check in.
-
A list of your current medications
Include dosages and note any medication allergies.
-
Recent laboratory or diagnostic work
Bring copies of any X-rays, blood tests or results from other facilities.
Ways You May Be Admitted
Pre-admission lets you pre-register before your visit. Call (716) 859-1234 and be prepared to provide your health insurance co-pays, insurance carrier verification, physician name, list of medications and form of payment.
Available hours: Monday through Friday, 8 a.m. to 6 p.m. On the day of your procedure, go directly to the information desk and staff will help you determine where to go.
You may be admitted to the hospital from the Emergency Department. After an examination and any blood or diagnostic tests, your doctor may treat and release you or determine that your condition requires a hospital stay.
- It may take some time before we can move you to an inpatient bed. Please know we are doing our best to expedite that process.
- The doctor who cared for you in the Emergency Department will update your doctor and the other doctors and nurses who will be caring for you.
- Additional tests may be required before you leave the Emergency Department.
Patients receiving recurring treatments for dialysis, kidney transplant evaluation, transfusions or OB-GYN care need only visit the Admissions Department on the initial visit. Additional visits for the same purpose simply require stopping at the front desk to receive a slip for your appointment destination.
A Health Care Proxy or Living Will (Advance Directives) lets you make your wishes known about who would act as your representative if you become unable to make decisions about your care. If you have prepared an Advance Directive, or would like to, notify a member of the healthcare team during your stay.
You will also be asked to sign an agreement regarding treatment consent and financial responsibility for expenses, including services not covered by your insurer. Please read it carefully.
