Healthy U: Medical Therapy Management on July 19

Join Jill Pogodzinski, Director of Community-Based Pharmacy for the Visiting Nursing Association of WNY, as she provides in-depth, medication-related education and advice to help ensure the proper use of medication.

Medication Therapy Management (MTM) includes several different ways that pharmacists can help patients get the most benefit from their medications and detect and prevent costly medication problems.  

Pharmacists review the patient’s medication list, make recommendations to start a new medication, stop a medication or change a medication, and follow up with both the physician and patient.

The purpose of MTM is to make sure every medication is at the right dose, to prevent side effects and to make sure the medication combinations are producing the healthiest outcomes for each patient.

MTM provided by pharmacists, the medication therapy expert, results in:

  • A review of all medications prescribed by all prescribers
  • Helping patients overcome barriers to taking their medicine as prescribed by the doctor
  • In-depth, medication-related education and advice to help assure proper use of medication
  • Collaboration with the patient, physician, and other health care providers to achieve each patient’s goals of medication therapy

MTM Services provide value, contributing to the safe, appropriate, and effective use of medications and offer value by:

  • Disease and symptom improvement, improved adherence, saved lives
  • Reduction in expenses associated with high drug copays, physician visits, urgent/emergency care visits and hospitalizations);
  • Reducing medication-related adverse events
