MyChart at Kaleida Health
MyChart is the secure patient portal that helps you stay informed, prepared and connected to your care at Kaleida Health. From scheduling appointments and messaging your care team to reviewing test results and managing medications, MyChart brings your health care together in one convenient place — on your phone, tablet or computer.
Why use MyChart?
Getting Started
Access MyChart
You can access MyChart:
- On a computer by visiting the Kaleida Health MyChart website
- On a mobile device by downloading the MyChart app from the Apple App Store or Google Play
After downloading the app:
- Open the app
- Search for Kaleida Health
- Log in or select Sign Up to create an account.
If you receive care at more than one organization that uses MyChart, you can securely switch between organizations within the app.
Sign up for MyChart
You can sign up in several ways:
- Receive an activation code during a visit
- Receive a link for sign up by email or text message
- Use online self-sign up to verify your identity and create an account
To activate your account:
- Verify your identity.
- Enter your activation code.
- Create a username and password.
- Select your notification preferences.
If you forget your username or password, use the “Forgot” links on the login page. After multiple unsuccessful attempts, you may be directed to contact MyChart support.

FAQs
Progress notes are shared by default with patients. Clinicians may choose to manually share additional types of notes.
Yes. Diagnoses are shared with patients unless they are manually unshared by a clinician.
All test results are released to MyChart.
- Normal labs are released immediately.
- Sensitive results, pathology, genetics, cytology and advanced imaging are released within three days.
All medications on a patient’s medication list are visible.
Patients and proxies may schedule visits including:
- New patient visits
- Follow-up appointments and new problem visits
- Physical exams
- Prenatal visits
- New gynecological visits
- Certain radiology and cardiology procedures
Proxies have full access to a child’s chart until the child turns 13 years old. After that, limited access remains for insurance coverage, payments, demographic updates, document signing and planning information.
MyChart is available in English and Spanish. Most navigation and system text is translated, though clinical messaging remains in English.
