The VNA of Northwest PA (VNA of NWPA) provides skilled medical and supportive care to patients in their homes throughout Northwestern Pennsylvania. As part of Kaleida Health’s home care network,VNA of NWPA delivers professional, coordinated services that support recovery, chronic disease management and independence at home.

The VNA provides a wide variety of services for patients managing chronic or complex medical conditions, or who are recovering at home.

In-home nursing services including wound care, medication management, disease monitoring, injections and patient education.

Physical, occupational and speech therapy services focused on restoring strength, mobility and independence.

Monitoring and education to help patients manage heart disease, diabetes, respiratory conditions and other chronic illnesses.

Hospice care
Hospice care provides comfort-focused support for individuals living with a life-limiting illness. When treatment shifts from cure to comfort, our goal is to help patients live as fully as possible, with dignity, peace and control over their care. The VNA of Northwest PA’s hospice team focuses on pain and symptom management while also supporting the emotional, spiritual and practical needs of patients and families. Care is delivered by a specially trained interdisciplinary team that may include nurses, home health aides, social workers, chaplains, therapists, dietitians, volunteers and physicians.

Assistance with community resources, discharge planning and support navigating medical and social needs.

Obstetric and maternal services
Home-based prenatal and postpartum care for mothers who need additional monitoring and support.

Pharmacy, infusion therapy and drug monitoring
Medication review and monitoring to ensure prescriptions are taken safely, prevent interactions and support effective treatment at home.

The Visiting Nursing Association of Northwest Pennsylvania serves the Bradford area and communities across the McKean, Western Potter, Northern Cameron, Northern Elk and Easter Warren counties.
What to Expect From Your Care
A licensed clinician evaluates medical needs, home environment and personal goals to develop an appropriate plan of care.
Care is tailored to each patient’s condition, preferences and recovery goals, and adjusted as needs change.
Our teams work closely with Kaleida Health hospitals, physicians and specialists to ensure continuity of care.
Getting started
Services may be requested by a provider referral, hospital discharge planner or by contacting our intake team. Referrals are taken 24 hours a day, seven days a week.
Request home health services
We accept most major insurance plans, including Medicare and Medicaid. Coverage varies by service and plan.
